Hemiarthroplasty / Internal Fixation for Fracture of Proximal Humerus

Indications:

  • Severe fractures of the proximal humerus
 In-patient from day 1 post op.
  • Mastersling with body belt fitted in theatre, the body belt remains on for three weeks with the patient wearing sling under their clothes for this time. They wear the sling during the night for 6/52.
  • Finger, wrist and elbow movements
  • Passive forward flexion in the scapula plane (within the limits defined at surgery & patient comfort)
  • Scapular setting

 

Day 21
  • Remove body belt, keep remaining sling on for another 3/52 but, can wear this outside of clothing.
  • Pendular exercises
  • Teach pendular dressing techniques
  • Passive abduction in supine.
  • Passive external rotation using walking stick within limits of pain
  • Active assisted flexion in supine within limits of pain and progress to sitting position as soon as the patient is able.

 

6-8 weeks
  • Wean off sling
  • All ADL as normal unless otherwise indicated at 6/52 review clinic
  • Isometric abduction, medial and lateral rotation
  • Static anterior deltoid strengthening (isometric flexion)
  • Stretching into flexion, medial and lateral rotation in supine within limits of pain.

 

Week 10+
  • Resisted deltoid and rotator cuff exercises through range

 

 No active or resisted medial or lateral rotation strengthening exercises until the tuberosities are healed (usually 6 weeks, be guided by surgeon)

 Improvement continues for 18 months to 2 years and the patients should continue exercising until their maximum potential has been reached

Return to functional activities

Driving                        After 12 weeks

Swimming                   Breaststroke:   12 weeks

Golf                            6 Months

Lifting                         Light lifting can begin at 12 weeks.  Avoid lifting heavy items for 6 months.

Return to work          Sedentary job: 12 weeks
